The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple co-morbidities. This position works in coordination with multiple PCPs and serves as the central link to an identified population of patients. Providence caregivers are not simply valued – they’re invaluable. Required Qualifications: - Coursework/Training: Graduation from an approved, accredited school of Nursing. - Upon hire: Washington Registered Nurse License - 2 years of work experience in an RN role. - Work experience revolving around medications and IV solutions in terms of purpose, administration, side effects and contraindications and administers according to established standards and provider orders. - Experience managing complex patients in an ambulatory setting.
