VNA Care is seeking a Clinical Admission Coordinator/LPN to join our Southborough team. Position Summary: Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient. Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers. Rate of Pay: $25.00 - $38.00, rate of pay is based on experience. Schedule: On-Site: Monday, Wednesday 8am-4:30pm; VNA Care, 333 Turnpike Road (route 9) Southborough, MA Remote: Tuesday, Thursday, Friday 8am-4:30pm: Weekend and Holiday rotation: Current LPN License required.
