This role manages the intake and admission process for new patients, conducts comprehensive assessments, and develops timely, individualized care plans. The RN provides both in-person and telephonic triage to evaluate patient needs, determine appropriate levels of care, and ensure effective coordination with families, healthcare providers, and community partners.
