In collaboration with the cardiologists, provides coordination of disease management activities in both the inpatient and ambulatory settings for a subset of high-risk heart failure patients. Triages patient and provider calls. Is a resource for staff in the management of this patient population. Coordinates care to ensure safe, effective, efficient patient centered care. Job Description: Provides front line support to patients and staff in the Advanced Heart Failure Program, including coordination of disease management activities in both the inpatient and ambulatory settings for a subset of high-risk heart failure patients. Collaborates with physicians, nurse practitioners, inpatient nursing staff, hospital staff, and the patient to coordinate all aspects of post-hospitalization care including: in-hospital patient teaching and evaluation prior to discharge, post-discharge phone call at 72 hours, medication titration, and medication reconciliation, scheduling of follow up appointment with HF staff post-discharge.
