Description The CSP-HI Case Management Project Coordinator is a dual-impact role combining expert case management with program coordination and administrative leadership. The CSP-HI funding stream through MassHealth supports housing stabilization and wraparound supportive services for individuals transitioning from chronic homelessness into permanent housing. As Victory Programs expands CSP-HI services across multiple programs, this role will play a critical part in developing and coordinating the agency’s CSP-HI operations. The Coordinator is responsible for key program functions including outreach, referral management, client enrollment, compliance tracking, interdepartmental coordination, and support of ongoing program implementation. Collaboration is central to this role, as the Coordinator will work closely with multiple agency programs within Victory Programs to identify eligible clients, facilitate seamless service delivery, and ensure continuity of care.
