Deputy Director, Care Transitions - ( 260002VQ ) Description The Executive Office of Aging & Independence (AGE) is seeking a mission-driven and dynamic individual to fill the Care Transitions Deputy Director position. The successful candidate must have specific program expertise in care transitions, be experienced in policy development, and program management and be able to communicate clearly conveying and articulating information in writing and orally to AGE managers, senior staff, external stakeholders, and providers. The Care Transitions portfolio includes the Community Transition Liaison Program (CTLP) and Money Follows The Person Demonstration (MFP Demo). These are two of several high visibility initiatives designed to support transition of consumers from institutional to community settings. The Deputy Director will be responsible for day-to-day oversight of CTLP and MFP Demo expanding the programs to meet current policy objectives which are geared to support an individual’s right to choose where they live with support in the community.
