Social Worker - Transitions Team Permanent: Grade C - £35,412 - £44,075 Working 36.5 hours per week: An exciting opportunity has arisen to join the Transitions Team. The team currently is subject of hybrid working arrangements, with an expectation of being office based (location will be determined by the Team Manager) 2 days per week, this being an essential requirement. Successful candidates will work with caseloads and citizens as part of its citywide service. The Transitions supports 17-25 year olds, who has a diagnosis of Learning Disability, Physical Disability and Autism as they transition to adulthood. We are a unique service working across Birmingham City Council and Birmingham Children’s Trust offering statutory support for young people leaving the educational or care system with care and support needs.
