Overview Big hearts like yours are needed within Benedictine! Our Case Manager-Social Workers are one of the first points of contact for our residents and their families during admission into our communities. Our compassionate and caring Case Manager-Social Workers help to make this emotional transition seamless and provides support throughout the process. Responsibilities: - The Case Manager-Social Worker manages the admission and discharge process as well as coordinates with internal and external customers. - The Case Manager-Social Worker develops and communicates with local hospitals/social workers and outlying areas in regards to availability at the community. - The Case Manger-Social Worker works with social services and the resident to determine a discharge plan.
