WHAT YOU WILL DO: The Hub Navigator is a non-exempt position responsible for providing assisted outreach services, in partnership with The Well for Life, to adults, children, and families. The Navigator may provide trauma-informed engagement, counseling, referral, and linkage to formal (community providers) and informal (family, friends, neighbors, school, etc.) resources to prevent ongoing client crises. - Provide an in-person response to referrals as required and within the periods designated by contractual requirements and/or program procedure - Engage with adults, children, and families, as well as their informal support networks (friends, relatives, neighbors, etc.) - Identify strengths, limitations, risk factors, and needs, using the required assessment tool to develop a Care Plan and coordinate services - Provide information, referral, and follow-up for appropriate ancillary community counseling, psychiatric services, financial, employment, parenting, housing...
