Position Summary: Responsible for the provision of social work to patients in need of these services throughout the cancer center. Works in active partnership with the MD’s, NP’s and RN’s and other disciplines to develop and implement a well-coordinated personalized plan to address patient needs across the continuum. The social worker assesses plans and facilitates options and services to meet an individual’s unique and complex needs.
