Job description

Our hospice team is looking for an On-Call RN/LPN to support patients Sunday - Tuesday nights. This on-call would begin Sunday mornings and cover Monday, Tuesday evenings. Daily Rate and Hourly for any in-person visits. Auburn Crest Hospice is looking for an On-Call Registered Nurse (RN) or LPN to join our Team. Duties include telephone triage for calls coming in from 5:00 pm to 8:30 am; Visits to patient’s homes, as needed; Charting and Communicating all call notes to case managers; Ability to work independently, but also as part of our interdisciplinary team.
