Sun River Health is a Federally Qualified, Non-For-Profit Health Center serving communities in Suffolk County. We currently have the need for a full-time Peer Outreach Worker to join our team located in Goshen, NY. Under the supervision of the Senior Director, the Peer Outreach Worker will work with the Assistant Director and Senior Prevention Counselor to conduct culturally sensitive, alternative venue outreach to engage in low threshold services for young men of color who have sex with men (YMCSM) and young people of transgender experience as well as youth at risk of HIV, STIs, medical, homelessness, public assistance, housing issues, Insurance, etc.. Engagement will be completed in person on the Mobile Medical Unit (MMU) as well as at the THRIVE by Safe Space and other events to provide low threshold services.
