Job description

Overview Assesses current and potential residents’ for placement throughout the village. Responsibilities: 1. Refer residents to appropriate health care services. 2. Investigate and direct patient inquiries or complaints to appropriate medical staff members and follow up to ensure satisfactory resolution. 3. Explain policies, procedures, or services to patients using medical or administrative knowledge. 4. Provide consultation or training to volunteers or staff on topics such as guest relations, patients' rights, and medical issues. 5. Collect and report data on topics such as patient encounters and inter-institutional problems, making recommendations for change when appropriate. 6. Identify and share research, recommendations, or other information regarding risk management. 7. Reviews, assesses and approves Health Care Center admissions. Contact resident insurance companies for approvals. 8. Attends and participates in resident assessments for transfers and new admissions 9. Communicate with discharge planners for daily census updates. 10. Assist in obtaining paperwork for Health Care Center utilizing Allscripts/ECIN programs to obtain hospital referrals. 11.
