Summary: The Navigator is responsible for guiding patients through the complexities of the healthcare system. Working collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients, the Navigator ensures that safety, best practice, and high quality standards of care are maintained across the continuum. This role provides support and resources to patients, including connecting patients to available services, navigates insurance authorizations, coordinates care between providers, and works with healthcare providers to ensure that patients are receiving the best possible care.
