Job Summary: Assists with helping patients and families navigate/access community services and other resources. Advocates for individual and community health needs and assists with providing direct services such as mammogram and diabetes screenings during community outreach activities. Supports staff in helping patients adopt healthy behaviors. Assists with promoting, maintaining and helping to improve the health of community patients and their families. Works under direct supervision.
