We are working closely with local authority in Herefordshire to assist with the appointment of a Senior Social Care Assessor , on a 3-months contract, highly likely to be extended at clients discretion. Rate of Pay : £17.85- £24.42 per hour Summary: The Senior Social Care Assessor plays a key role in delivering high-quality, person-centred Adult Social Care services within the Community Wellbeing service. Working as part of a multidisciplinary team, the postholder will undertake strengths-based assessments, care management, reviews, and mental capacity assessments to support adults, young adults transitioning into adult services, informal carers, and vulnerable individuals to maintain independence, wellbeing, and positive outcomes.
