Position Overview: As an Advanced Practitioner at Community Care Family Practice Colonie, you will deliver full-spectrum family medicine services in a collaborative, team-based environment. You will practice to the full extent of your license with strong physician support, integrated clinical services, and scheduling flexibility designed to promote work–life balance and long-term professional satisfaction. Responsibilities: Conduct comprehensive patient evaluations and manage acute and chronic conditions. Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed.
