Department: UHC: Phys Med Grp - RN Rheum Job Summary: The Patient Navigator coordinates access of patients through the healthcare system from initial diagnosis or point of referral. Gathers demographic, employment, and insurance data necessary for registration and verifies insurance coverage and benefits. Gathers needed medical information, including diagnostic procedures, operative notes, and progress notes. Functions as a liaison between patients, families, outpatient clinics, primary care physicians, hospital base, and other support services. Requesting authorization for specific services as ordered by provider(s). Assists with patients in clinic environment. The duties outlined in this Job Description will be an important part of our Hospital Outpatient Department strategy for the delivery of care that is consistent with our mission, vision, and values.
