Samaritan Daytop Village, serves over 33,000 New Yorkers annually within your neighborhoods and communities so our success depends on those we employ. The Role: The Outreach worker is essential in locating, engaging and enrolling assigned Health Home eligible persons who have been identified by the NYS Department of Health to be in need of comprehensive and coordinated social services. Outreach activities are defined within reason as electronic searches, correspondence, telephone calls, home visits, field visits and/or agency visits. Transportation is provided for all outreach activities or is reimbursed within the parameters of participating Health Home providers.
