Master•Care, Inc. is a Managed Services Organization (MSO) created exclusively to bridge medical and non-medical services under California’s new CalAIM program. Enhanced Care Management, Housing Navigation, and Nursing Facility Transition are just a few services we provide. POSITION SUMMARY: A Master•Care Care Navigator provides Care Management to patients in a non-clinical setting according to the “Master•Care Plan.” The Master•Care Plan is a comprehensive roadmap that incorporates the physical, behavioral, social, environmental, and financial well-being of our patients.
