Job description

Patient Safety/ Risk Coordinator - Establishes and coordinates a System wide patient safety program that fosters ongoing improvement in patient safety and complies with accreditation and regulatory standards. - Works with executives, management, hospital and medical staff to develop a culture of safety and medical error prevention. Investigates safety related incident reporting, escalating as appropriate. - Conducts risk assessments, collecting and analyzing documentation, statistics, and reports; Responsible for coordinating activities resulting in accreditation; Drive Root Cause Analysis and Failure Mode Effect Analysis processes. - Prepares and presents reports on patient safety metrics, compliance, and outcomes to leadership.
