Employment Type: Full time Shift: Day Shift Description: Monday - Friday Day shift Position Purpose: The Wound Ostomy Nurse is a Registered Nurse specialist that provides direct patient care, to coordinate, deliver, and evaluate patients with wounds and ostomies, or are at risk for skin breakdown. Other duties include performance improvement, data collection and working with supply chain management to consult on materials. Specialized training includes certification in wound and ostomy. What you will do: - Responsible for collaboration, communication & facilitation of optimal provision of care supporting management of wound, skin and ostomy care. - Consult to recommend best practice for complex wounds, ostomies and preservation of skin. - Data collection and reporting quality outcomes to performance improvement. - Monitor quality metrics specific to the department & ensure complete & accurate documentation in the patient record. - Works with interdisciplinary team of hospitalist physicians, clinical affiliates, nursing, care managers, nutrition, Rehab services, and pharmacy. - Utilize clinical knowledge, critical thinking skills & the principles of case management & adult education...
