Description About the Role: The H.U.F. Center is seeking a compassionate and reliable Case Manager to support adults with mental health and co-occurring disorders in a psychosocial day support setting. As a Case Manager with Hope, Unity, and Freedom (H.U.F.) Center, you play a vital role in supporting adults with primary mental health diagnoses, co-occurring disorders, and/or substance use challenges. This position focuses on providing psychosocial day support services that promote independence, stability, and overall well-being. This is a hands-on, highly interactive role where you’ll work closely with members throughout the day—providing direct support, facilitating interventions, and ensuring all services are clearly and accurately documented. The role requires consistency, strong interpersonal skills, and the ability to work with a diverse and sometimes high-acuity population.
