JOB DESCRIPTION: SUMMARY OF POSITION Responsible for providing supports and services to clients and their families primarily in the client’s natural environment to facilitate the achievement of personal growth goals. This involves providing individual and group therapeutic interventions, using an integrated health approach, that facilitate personal growth, skill building, identification and use of natural supports and community resources. Responsible for assisting with assessment and personal growth planning, linking clients to appropriate services, advocating on their behalf, providing support, monitoring effectiveness of services and providing crisis services as needed.
