This remote position offers flexibility and the opportunity to make a significant impact on the lives of patients receiving home health and hospice care. Responsibilities: Conduct virtual nutritional assessments of patients via telehealth platforms (e.g., Teams, Google Meet, Zoom, FaceTime, phone calls). Evaluate nutritional risk factors and craft personalized nutrition care plans to manage chronic conditions, prevent illness, and maintain comfort. Review patient information, including H&P/face sheets, current progress notes, intake, anthropometrics, etc., provided by the clinical team.
