The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West, Great South, Upper wheatbelt and Peel Regions of Western Australia. SWAMS delivers a wide range of Services including: GP Clinics, Primary Health Care, Counselling, NDIS and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development. About the role: This part time role is working within the Maternal Child Health team to support women and their families. This role will support them during antenatal, birthing and postpartum care. The ORCHID program is a family centered program for diabetes in pregnancy. - Coordinate and support education about antenatal and postpartum testing for diabetes and future risks for developing diabetes - Assist ORCHID Program participants and their support people to understand and follow an individualised diabetes care plan, including any recommended health behaviour changes - Support ORCHID Program participants’ and their support person’s use of technology to manage diabetes - Faci...
