(Fully licensed) (New Employee is defined as someone who has not been employed with Rogers for more than six months) The Social Services Case Specialist develops, coordinates and delivers all direct case specialist services to patients and their families assigned to him/her, and performs other duties as directed. Job Duties & Responsibilities: Provide social services to patients and their families/significant others: Complete assignment and documentation within required time. Communicate professionally and accurately.
