Position Summary The Registered Nurse Coordinator in this position is responsible for the care coordination of the Structural Heart Program as part of a team of nurses, cardiologists, and cardiac surgeons. Essential Duties and Responsibilities: - Receives and reviews patient referrals for appropriateness, reviewing with an attending provider as needed for any structural intervention. - Establishes contact with patient/family to explain process and work-up and testing schedule. - Facilitates obtaining all outside imaging, scheduling screening procedures and structural heart clinic visit and any additional testing. - Performs functional assessments and frailty testing for this patient population. - Prepares all testing available for review by cardiologist and cardiac surgical team at clinic visit. - Participates in development of treatment plan and performs patient and family teaching. - Facilitates direct contact between cardiologist and referring physician regarding treatment plan. - Uploads required data to appropriate vendor and maintain communication with device representatives. - Facilitates scheduling and authorization of structural heart procedures. - Contacts the patient 48-...
