Cohen Military Family Clinic. This position serves as a key point of contact for clients by assisting with intake coordination, referral screening, care navigation, resource linkage, outreach activities, and collaboration with community and military stakeholders. The role supports continuity of care and helps ensure clients receive timely access to appropriate behavioral health and supportive services. QualificationsESSENTIAL JOB RESPONSIBILITIES: Screen referrals for eligibility and appropriateness of clinic services according to program guidelines. Provide case management and wrap-around support services to clients and families. Connect clients with community resources including housing, employment, financial assistance, transportation, benefits, medical care, and social services as appropriate.
