This role focuses on patient engagement, medication management, and collaboration with pharmacists to improve health outcomes in all settings including but not limited to community, long-term care and specialty services. Seeking candidates interested in expanding professional role beyond dispensing into patient-centered care and clinical service delivery. MAJOR RESPONSIBILITIES: - Assist pharmacists in the delivery of clinical services, including but not limited to: - Medication Therapy Management (MTM) - Comprehensive Medication Management (CMM) - Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs) - Immunization support and documentation - Point-of-care testing (e.g., A1c, lood glucose, cholesterol, flu, strep) - Remote patient mon...
