Home Care Assistant, STNA Ashtabula County - ( 2600028J ) Description: - Provides patient care, under the supervision of a Case Manager: Nurse or therapist. - Provides personal care/companionship as directed by plan of care - Provides light housekeeping/ light meal prep tasks as directed by plan of care - Effectively communicates both verbally and in writing - Initiates and maintains positive relationships with patients/customers and co-workers - Observes and reports patient status changes including skin and psychosocial - Takes responsibility for self-development and supports a learning environment - Displays commitment to the mission of UHHS/UHHCS and its values - Performs other duties as assigned. - Complies with all policies and standards. - For specific duties and responsibilities, refer to documentation provided by the department during orientation. - Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. - Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. - Monday-Friday. - Part time 20 hours per week. - 100% travel to the patient...
