Provides supervision, leadership, and guidance to staff of HCS, TxHml, GR, PASRR, CFC, ECC, CLASS, Access and Intake, Enrollment, and CLOIP service coordinators. Provides direction under the Service Coordination functions rule in the Texas Administrative Code, 40 TAC, Chapter 2, Subchapter L: Service Coordination for individuals with IDD. Assists with the TST/MFP program. Supervision of service coordinators to assure that the development of each assigned consumer’s Person Directed Plan is individualized, outcome orientated and meets HHSC guidelines. Data entry for in-coming and out-going transfers. Monitors service coordinator’s documentation. Provide monthly and quarterly reports to supervisor and partnering agencies.
