Employment Type: Part time Shift: Day Shift Description: The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes. What the Nurse Navigator will do: - Completes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and - functional status in collaboration with multidisciplinary team members. Includes assessment of past and present treatment course. - Assists in organizing and coordinating timely patient appointments. - Understands and develops goals, treatment plan and provider options in concordance with evidence based standards and clinical trials. - Works directly with physician lead to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of care. - Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes an...
