You will be serving NDIS Participants in and around FRANKSTON (Victoria) About the Role: As a Support Coordinator, you will work alongside NDIS participants to turn their plans into action. You’ll build meaningful relationships, guide participants to make informed choices, and connect them with the right services. Your work will directly shape participants’ independence, confidence, and quality of life. You will: Transform participant goals into clear, achievable plans. Connect participants with the right providers, supports, and community opportunities. Monitor progress, track outcomes, and ensure supports are delivered effectively. Build and maintain strong relationships with Local Area Coordinators (LACs), providers, and key stakeholders to enhance participant outcomes and drive referral pathways for LifeFul.
