Licensed Social Worker for Skilled Nursing Facility Department: Quality Care Rehab: Employment Type: Full Time: Location: Fair Haven: Description: A Licensed Social Worker (LSW) in a skilled nursing facility (SNF) provides comprehensive psychosocial assessments, care planning, and therapeutic interventions to support residents' mental and emotional well-being. They facilitate admissions/discharges, advocate for resident rights, and mediate between families and the interdisciplinary care team. Key Responsibilities: Psychosocial Assessments & Care Plans : Conduct comprehensive assessments upon admission and develop individualized, 90-day, or annual care plans. Discharge Planning : Coordinate with insurance managers, families, and community resources to manage safe discharges. Resident Advocacy & Rights : Ensure compliance with federal/state regulations, promote resident dignity, and protect rights. Family/Resident Support : Offer crisis intervention, counseling, and education to help residents adjust to long-term care.
