Organization: SCAH-Sutter Care at Home - Valley Position Overview: Responsible for providing nutritional consultation services to patients, referral sources and staff in order to facilitate and maintain enteral and Total Parenteral Nutrition (TPN) according to initial screening protocols for enteral and TPN or at the request of the physician or case manager to ensure that these patients who are at risk have been assessed and treated according to nutritional standards of care for enteral and TPN therapies. Dietary plans are developed with consideration of patient/family emotional, social, environmental and economic patterns. EDUCATION: - Bachelor's: BA or BS degree in Nutrition, Dietetics or related field - Other: Completion of an American Dietetic Association approved internship program. - Other: Continuing education hours as required by the Commission on dietetic Registration and other subspecialty credentialing agencies.
