PROGRAM: This position is for the Back Home SSVF (Supportive Services for Veteran Families) program which is a Rapid Rehousing program that utilizes a 90-day model to find and stabilize housing for veterans experiencing homelessness or at risk of homelessness. Primarily focusing on housing case management, but also offering wrap-around case management services assisting to connect veterans to Public Benefits, VA benefits, Health referrals, and Financial health referrals.
