Job DetailsJob Location: 2400 Moorpark Avenue, Suite 300 - San Jose, CA 95128Salary Range: $33.83 - $39.00 HourlyJob Purpose Under the direction of the Program Manager, the Program Lead provides support and coordination of programmatic activities in addition to delivering direct services. This position is responsible for supporting program operations, leading projects, and providing an additional level of support for ECM Case Managers. Duties and Responsibilities: Conducts outreach to engage and enroll eligible community members to the Cal AIM ECM Program. Conducts outreach to engage and schedule new patients assigned to clinic Prepares, assigns and tracks lists for team to contact patients for outreach. Completes high-risk behavioral health and housing assessments with client to identify need areas. Develops and maintains an individualized care plan for each client. Track program activities/outcomes, and provide progress reports. Supervise, evaluate, and provide performance feedback to program staff on a regular basis.
