The Care Coordination Supervisor is primarily responsible for ensuring adherence to the high-fidelity wraparound model of care coordination, ensuring quality care for children and families/caregivers representing diverse socioeconomic, racial, ethnic, and cultural backgrounds. The supervisor has a thorough understanding of local community resources and are skilled at developing and maintaining working relationships with staff, clients, and community partners.
