Description JOB DESCRIPTION: Assesses, develops, implements and evaluates nutrition care for patients using Nutrition Care Process. Complete competency of physical assessments on patients. Provides care independently. Provides nutrition counseling to patients and caregivers based on identified needs. Participates in medical rounds, discharge planning and patient care conferences. Completes nutrition transplant orientation and is able to provide nutrition care per CMS guidelines for patients in all 3 phases of transplant. Provides on call patient care as scheduled. Provides weekend and holiday coverage as scheduled. Assess, implement, manage or adjust diet prescription for oral/enteral/parenteral nutrition, as appropriate for patients medical condition. Makes appropriate use of available education materials. Resource to FNS staff for evaluation and recommendation of nutrition products and supplies. May serve on FNS/hospital wide committees. Effective time management in organizing workload and accomplishing tasks. Identifies problems, collects data, and takes corrective action for new and/or assigned performance improvement projects.
