Role: Director, Project Based Housing, Jail Programs Reports to: Senior Director, Jail Programs (ODR) Program: 1602-DHS ODR Department: Permanent Housing Location: Sherman Way Van Nuys - 13561 Sherman Way, Van Nuys, CA 91405 Setting: 100% Onsite Schedule: Monday - Friday, 8:00am - 4:30pm Status: Full-time/Exempt/Management Benefits: Medical, Vision, Dental, Life Insurance, 403(b) Retirement plan, Employee Assistance Program (EAP), etc. Openings: 1 Job Description Summary: The People Concern has partnered with the Department of Health Services (DHS) and the Office of Diversion and Re-entry to provide housing and Intensive Case Management Services (ICMS) to persons being released from jail with a prior history of homelessness. Participants are linked to the ODR program while they are in jail. Once released, Case Management staff will assists individuals matched to an ODR Project Based Housing site with the lease up process. Once housed, the goal is to help individuals with housing stability and retention by providing intensive services.
