This is NOT a remote position Title: Weekend Home Health RN (Field) Schedule: Per Diem (12 RVE p/week) must work Saturdays and Sundays 9am-5:30pm Locations: Area #45 covers the following cities: Agoura Hills, Calabasas, Moorpark, Newbury Park, Oak Park, Simi Valley, Thousand Oaks and Westlake Village Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm in San Ramon (onsite) Position Summary: The Home Health RN monitors a patient’s overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager, and works independently to deliver quality healthcare to their patients.
