Key Responsibilities: - The Platform Coordinator will need to have organizational skills with the ability to effectively prioritize and complete tasks to support Care Coordinators with platform referrals. - Coordinates to process patient referrals from Navi, Allscripts, Epic NCH, and Power chart, as well as other referral platforms. - Must be detail-oriented, possess the ability to multitask, and be open to cross-functional training in all departments. - Displays a high level of patient focus that works directly with Care Coordinators, Case Managers, Physicians/PCP(s), insurance representatives, outside vendors, and other medical personnel. - Pulls referrals off platforms to accept/decline, Reviews documents, confirms, and processes the referral thr...
