Employment Type: Full time Shift: Day Shift Description: RN Case Manager in our Psychiatric Medical Unit. Full Time Mon-Fri day shift Position Summary: Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care. What the case manager will need: Education - BSN or ADN with BSN to be obtained within 4 years of hire License: - RN License in the State of Michigan - Case Management Certificate preferred Experience: - 2-5 years of medical / surgery or chronic disease management - Previous Psychiatric RN experience strongly preferred - Previous Case Management experience preferred What the case manager will do: - Clinical Expertise and critical thinking skills. Provides a competent patient assessment by obtaining and interpreting data, making appropriate decisions based upon knowledge and experience and, if applicable, adapting plans accordingly.
