Schedule Full-Time: 40 Hours/Weekly: 8:30am - 4:30pm, Monday through Friday. Hours may vary based on patient and business needs. General Summary: Provides direct patient care and drug therapy management services for patients with specialty conditions upon provider referral. Working under a written protocol, the pharmacist partners with providers to manage a patient's drug therapy, optimize outcomes, improve access to specialty medications, and ensure safe, cost-effective medication use. Remote Work Capable: Essential Functions: - Evaluates appropriateness of drug therapy for patients with specialty conditions referred to the WellSpan Management of Drug Therapy (MDT) Clinic (e.g., rheumatology, dermatology, endocrinology). - Adjusts drug therapy and order laboratory tests or other diagnostic tests necessary in the management of drug therapy under Collaborative Practice Agreement (CPA) or written protocol in accordance with state law. - Monitors treatment efficacy, therapeutic outcomes, safety parameters, and adherence for patients receiving biologics and other specialty agents. - Identifies and resolves drug therapy problems throughout the course of medication therapy. - Provides pa...
