**$10,000 Sign on Bonus!** BAYADA Home Health Care is currently seeking an experienced Occupational Therapist, OT, for a full time or per diem opportunity performing home health visits . This office works with adult and geriatric patients on a per visit basis in senior living communities throughout Somerset, Middlesex and Monmouth Counties . As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home. Occupational Therapist, OT Responsibilities: - Perform outpatient therapy evaluations in order to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilities - Professionally operate at top of licensure utilizing current research-based approaches to care in order to maximize functional independence for your clients - Work closely with your home health therapy partners to appropriately transition care from home health to outpatient services in order to achieve client centered goals in their living envir...
