Our Company ResCare Community Living: ResCare Community Living is now part of Sevita Overview: *2,500 Sign-On Bonus* Plans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients. - 12-hour rotating shifts- days 7am-7pm (3on, 2off, 2on, 3off) - Every other weekend off - No On-Call time required - No uniforms required - Small facility with only 20 clients - Administer medications - Coordinate physician and health care services for people served - Conduct health care assessments and write health care plans (for assigned individuals only- typically 6-8 clients) Responsibilities: - Provide 1:1 care for client always following client’s plan of care while maintaining client's dignity and independence - Monitoring clients’ vital statistics and reporting abnormal findings to the case manager and physician assessments - Performs medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results - Complete routines as establi...
