RN Liaison

Job description

Reporting to the Manager of RN Liaisons, the RN Liaison facilitates referrals to hospice, assists in developing relationships with business partners in the community, and performs timely, individualized, accurate assessments for patients referred to the organization. The RN will represent GHS in a professional manner and is committed to delivering the highest quality of care possible as identified in the GHS mission statement. SHIFT: 4x10 hours, rotating weekends, work 1 minor and 1 major holiday annually EDUCATION AND QUALIFICATIONS: - Currently licensed in the State of Florida as a Registered Nurse - A minimum of two (2) years clinical experience working in and acute care settings; one year of homecare and/or hospice experience preferred. - Active CPR Certification - Must maintain current Florida driver’s license - Must maintain valid automobile insurance coverage ESSENTIAL JOB RESPONSIBILITIES: - Maintains professional and clinical knowledge of provider services.
