About the role We are seeking an Occupational Therapist to join our small rural health service, playing a vital role in supporting older people across the Tallangatta community and surrounding region. This position has a strong focus on community and aged care services, supporting older adults to remain safe, independent, and engaged in their own homes for as long as possible. Working as part of a close-knit multidisciplinary team, you will manage a predominantly aged care caseload across home and community settings . The role includes comprehensive functional assessments, care planning, and delivering practical interventions such as equipment prescription, ADL support, home modifications, falls prevention, and cognitive screening. A key component of the role is supporting clients to navigate aged care services, including Support at Home funding pathways, while working closely with families, carers, and local providers to coordinate care and optimise outcomes.
