This is a professional position providing confidential case management/guidance to vulnerable adults the following service programs: 1. Protective Intervention (PI), a. Placement, b. Supportive Services, c. Short Term Case Management 2. Protective Supervision (PS), 3. Home care for the Disabled Adult (HCDA), 4. Community Care for Disabled Adults (CCDA) Prepares and maintains detailed client assessments and service plans for each client and records/documentation in accordance with department and program policies and procedures.
